ProActive Orthopedic and Sports Physical Therapy is an outpatient rehabilitation clinic which emphasizes individualized, hands-on care.

The goal of ProActive Orthopedic and Sports Physical Therapy is to provide residents of Oregon and SW Washington with a locally owned and managed physical therapy facility. We currently have locations in Gresham, Oregon City, Canby, Vancouver, and Myrtle Creek.

Patients treated by a licensed therapist or licensed assistant each office visit. Extended office hours to accommodate the working patient Monday through Friday 7:30am to 6:30pm. Immediate appointment.		Patients seen within 24 hours of referral or on a walk-in basis when requested. Personalized Evaluations and Plans of Care. No computerized templates. Initial evaluation and progress reports promptly provided in addition to ongoing verbal communication. Most Insurances accepted.
